All new clients must complete medical information and consent forms* prior to receiving massage. These forms may be updated periodically, especially if there is a change in health or a medical condition. This information is used to help plan safe and effective massage sessions.
Should the therapist determine that a real or potential contraindication for massage exists, the therapist may decline to accept a client, may discontinue services (temporarily or permanently), or request that a medical release be obtained from the treating health care provider.
Sexual harassment of the therapist, including but not limited to, inappropriate touching, comments, gestures, or other threatening behavior will not be tolerated. Only legitimate, professional relaxing and therapeutic massage is provided.
A notice of 24 hours is expected should the need arise to reschedule a massage appointment.
*The Department of State Health Services Massage Therapy Licensing Program mandates that a massage therapy document meets the requirements established by Rule 25 TAC State Statute 140.304. Consultation Document.
